The Bushland Health Group NP Palliative Care Model has delivered quality care and a rapid response to end of life symptoms in a Residential care environment thus achieving the standard of a dignified death in their home environment in their own Manning Valley Community. Providing a high standard of Palliative Care and enhancing service provision for all Australians is the aim of the National Palliative Care Strategy 2010. Bushland Health Group a regional community owned organisation operating three Residential Aged Care Facilities (RACFs) employs a full time Nurse Practitioner (NP) in Older People Nursing and who oversees the provision of palliative care across its 263 Residential aged care beds. The introduction of the Residential Aged Care Palliative Approach Toolkit and End of Life Care Pathway (EoLCP) was the catalyst for the Organisational review of its Palliative Care service. Based on extensive feedback, the organisation considers the NP Model of Collaborative Palliation is achieving the goal of a ‘dignified death’ demonstrated over time and across the Residential aged care facilities.
